Nature of award (i.e. scholarship, tickets, cash, plaque, etc.)

Plaque or certificate (not to exceed $25) and a gift, such as a bookstore gift certificate (not to exceed $75) presented by the Health Science Library Association chair at the annual spring health science library conference.

At the discretion of the Iowa Health Science Library Association Executive Committee.

The Iowa Health Science Library Association Executive Committee accepts nominations from its membership throughout the year and determines award recipients by Executive Committee vote.

Award is conferred upon individuals who have made outstanding contributions to health science library services or who have advanced the profession of health science librarianship within the sate of Iowa. Recipient will have influence and reputation beyond the local community. Award may be given either to ILA members or to those who are not members of the Association.Award may be given to an individual or a group of individuals.

Process for notification

Identity of recipient(s) remains confidential to IHSLA Executive Committee until announcement is made at the spring conference. Plaque and gift is presented to the recipient(s) at the spring conference.

Section 2. Iowa Health Science Library Association Annual Midwest Chapter/MLA Conference Scholarship Award-Official name (Midwest Chapter Conference Award-common name)

Nature of award (i.e. scholarship, tickets, cash, plaque, etc):

This award is given to a member to promote attendance at the Midwest Chapter Annual meeting and encourage membership in ILA for health science librarians.The award is intended to be applied toward the conference registration fee and travel, hotel, and meal expenses, or any combination of these. Scholarship winners will be reimbursed (up to the $1200 amount) for scholarship expenses after the conference.

A maximum of one scholarship is awarded each year.

The funding source of this award is income from the Iowa Health Science Library Association Spring Conferences and/or subdivision pre-conferences.

Process for nominations (including dates/time frames)

Applicants must be members of ILA/Health Science Library Association, working in an Iowa health science library or in an Iowa academic library in a college or university with at least one health science program. Previous scholarship recipients are not eligible to receive the award more than once every 10 years. A call for nominations will be sent out through the Iowa Health Science Library listserv at the time of the annual spring IHSL meeting announcement.The scholarship application deadline date is at least three months prior to the fall Midwest Chapter conference and winners will be notified at least one month prior to the conference. For example, the Midwest Chapter Conference held on October 3-6, 2010 will have July 3, 2010 as a deadline date for scholarship applications and the winner will be notified by September 3.

Applicants must complete and submit an application form which includes an essay  of no longer than 500 words that describes the applicants’ interest in the scholarship and how the scholarship will benefit the member’s professional development. Scholarship winners are selected by the Iowa Health Science Library Association Executive Committee on the quality of the essay. Each member of the Executive Committee blind ranks the submitted essays and the scholarship winner is the applicant that receives the highest group ranking. A member of the committee may submit an application but would then refrain from participating in the vote. The chair can appoint an IHSLA member who has not submitted an application to substitute as a voting member for the abstaining executive committee member.Winners will be notified at least one month prior to the conference.

Winners will be notified at least one month prior to the Midwest Chapter/MLA conference either by email or by phone. If the winner declines the scholarship award or cancels attendance to the conference, the applicant with the next highest group ranking will receive the scholarship award.

Requirements of winners, if any, with details as to compliance follow-up procedures:

Winners are requested to write a short article about their conference experiences for the Midline newsletter following the conference.  Award winners will be reimbursed for scholarship expenses after the conference.They will submit an itemized list of expenses with receipts along with a brief report to the Chair of the Iowa Health Science Library Association for approval and submission of payment voucher to the ILA Office.

Section 3. Iowa Health Science Library Association Memorial Award—official name
(Memorial Award—common name)

Nature of award (i.e. scholarship, tickets, cash, plaque, etc.):

This award is to honor the memory of a deceased librarian who has served in a health science library within the state of Iowa. This librarian need not have been a member but someone who has contributed to Iowa health science libraries. Upon the death of a current or former health sciences librarian, a gift not to exceed $400 in cost will be made to the library where that person most recently worked to be used for the purchase an item of need. Appropriate gifts would be an item for the library collection, a piece of equipment or décor, or other item determined by the receiving library director. A memorial plaque, at an additional cost, not to exceed $50.00 will accompany the gift. The award will be announced at the Health Science Library Association Spring Conference.

Any Iowa Health Science Library Association member who is notified of the death of a former or currently working health sciences librarian, should inform the chair of the Iowa Health Science Library Association. The chair will contact the director of the library where the deceased worked who will identify a needed library item. The Iowa Health Science Library Association Executive Committee will approve the purchase of the item.

The director of the library where the deceased librarian worked (or a representative of the director) will be invited to the Iowa Health Science Library Association annual spring conference to receive the award.

Section 4: Iowa Library Association Dues and Fall Conference Scholarship (official name)

Up to $500 scholarship, expenses and ILA dues for one year.

Subdivision dues and other subdivision income

Eligibility

Anyone currently working in an Iowa health sciences library is eligible to apply.

Process for nominations (including dates/time frames)

Availability of the scholarship is announced via health sciences libraries subdivision listserv no later than July 15th of each year, and in the Catalyst.
Self-nomination or nomination must be submitted by August 1st for consideration.
Applications are submitted by e-mail to the subdivision chair. Applicant will be asked to supply name, position title, name of library, e-mail address, and phone number.

Process for selecting winner (including dates/time frames)

A member of the Iowa Health Sciences Library Subdivision Executive Committee will draw recipients’ names from among applicants. Drawing will be held and winners will be notified by August 15th by the Subdivision chair. If a winner cannot attend the conference, an alternate will be drawn from among the remaining applicants.

Winners will be notified immediately after drawing of names by e-mail or phone call.

Requirements of winners, if any, with details as to compliance follow-up procedures

Winners must use the money to pay for an annual ILA membership, including the IHSLA subdivision and attend ILA Annual Conference in year for which scholarship is awarded and present receipts for reimbusement.

Section 5: Iowa Health Science Library Association Leadership Institute Scholarship -official name (common name  IHSLA Leadership Institute Scholarship)

Nature of award (i.e. scholarship, tickets, cash, plaque, etc.)

This award is given to encourage membership and promote leadership within the Iowa Health Science Library Association. The award will cover the registration and travel expenses of up to $300 for an Iowa Health Science Library Association member who is accepted to the Leadership Institute. Scholarship winners will be reimbursed (up to the $300 amount) for expenses after the Institute.

Frequency and/or number of awards given

If funds allow, A minimum of one scholarship is awarded every other year to coordinate with Iowa Library Association’s Leadership Institute. However the number of Leadership Institute Scholarships given each time may vary depending upon available funds and approval by the IHSLA Executive Committee.

Applicants must be members of ILA/Iowa Health Science Library Association and must have applied for (and ultimately be accepted to) Iowa Library Association’s leadership Institute. A call for nominations will be sent out through the Iowa Health Science Library listserv and newsletter at the time applications are accepted for the Leadership Institute.

Process for selecting winner (including dates/time frames)

A copy of the Leadership Institute application along with the acceptance notification should be submitted to the subdivision chair within ten days of notification of acceptance to the Institute. The Executive Committee will vote to determine the winner based on the application content; with preference given to any applicant who is a subdivision officer.  Applicants who are on the Executive committee would not vote.

Winner will be notified of the award by e-mail ten days after submission of the application to the subdivision chair.

Requirements of winners

Section 6: Iowa Library Association and Iowa Health Science Library Association Subdivision Dues Scholarship (common name ILA/IHSLA Dues Scholarship)

Nature of award (i.e. scholarship, tickets, cash, plaque, etc.)

This award is given to a new, lapsed or current ILA member to promote ILA and IHSLA membership for health science librarians. The award is intended to be applied towards the fees for one year of ILA and IHSLA membership. Scholarship winners will be reimbursed the amount of their ILA dues based on their current annual salary (not to exceed the maximum dues requested on the ILA Membership Application), as well as be reimbursed the currently listed dues for the Health Sciences subdivision.

Frequency and/or number of awards given

A maximum of two scholarships are awarded each year.
